Our latest case study highlights how a local Volunteer Centre quickly set up a community-asset based social prescribing project in response to a local need.
Karen Chillman, Head of Volunteering at Volunteer Centre Croydon, shares her story on how we worked together to set up the Connect Well Croydon social prescribing service to support local people into local services, and help address some of the wider determinants of health.
Using a volunter-led model, Karen and her team of Connect Well Guides are providing 1-1 support to people and help them navigate to the right support and service for them.
“Our Connect Well Guides are just ordinary people – having a conversation with someone about what they’re interested in, and then using the system to quickly find something suitable and make a direct referral on their behalf.”
The project is quickly gaining recognition and engagement from health professionals and community partners, such as the Council’s Youth and Neighbourhood teams and Police Community Support Officers.
